MyYLife Survey for Parents of Children in Grades K-5

This survey asks questions about your child’s life, especially your child’s life at the YMCA. This survey asks about values, volunteer and service activities, and your child’s interest in learning, both in and outside of school. It also asks questions about things your child does to build her or his body, mind, and spirit, and her or his activities at this YMCA.

The Y will use the answers of many young people and their parents, including your answers, to improve what goes on at the Y.

You will need to take the survey all at one time. It should take about 15-20 minutes.

Some questions ask how much you agree or disagree with a statement, or how true something is for your child. Other questions ask how often your child does or feels something. Sometimes, you might not be exactly sure of your answer. Just give the answer you think is most honest or accurate for each question. The survey is confidential and anonymous—no one will know how you responded.
